Please accept my official request to reactivate my revoked membership with the United States Golf Teachers Federation. With this reactivation, I understand that I must abide by the USGTF Code of Ethics and bylaws of the organization.

I understand that I may use the USGTF name and/or logo for any promotion purposes as long as I remain a member in good standing and will represent myself and the organization in a professional manner at all times.

I agree to pay the $500 reactivation fee in full as well as any past due fees in my membership account.
